Request Your Credit Report

If you have recently applied for credit and had any adverse action taken against you based on information provided by Upstream Logix, you are entitled to receive a copy of your Upstream Logix credit report free of charge. Additionally, you are able to request one free copy of your Upstream Logix credit report annually. 

To request a copy of your Upstream Logix credit report, please download the request form by clicking on the Get Form button below.  Please complete the form in its entirety including your signature and the current date.

For your protection, we will need to properly identify you prior to sending you your consumer report. Along with your report request, please send to us a clear copy of both sides of your Driver's License or government issued identification card. 

Alternatively - you may send any 2 of the following types of identification information: 

- A copy of a recent (no less than 60 days old) cable, utility or phone bill, with a matching address of request. 

- A copy of your Social Security card. 

- A copy of your birth certificate. 

- A copy of your US passport (picture page only). 

- A copy of your voided consumer check with matching address of request. 

- A copy of your Alien Registration Card. 

PLEASE NOTE: If any of information you provide is different than the information shown on your Driver’s License or government issued ID card, then also include one of the following dated within the last 60 days:

· Utility Bill

· Cell Phone Bill

· Cable Bill 

· Military Orders

The Fair Credit Reporting Act (the “FCRA”) allows Upstream Logix to include your name and address on prescreen lists used by lenders for sending you unsolicited offers of credit.  If you would like your name and address to be removed from such prescreen lists for 5 years, please call us at: 813-331-5189.

If you would like to permanently opt-out from such prescreen lists, please mail us directly with your request to:

Please include the following information:

• Your first, middle and last name (include Jr., Sr., III, etc.)
• Your current address
• Your previous address (if you have moved in the last 6 months)
• Your Social Security number
• Your date of birth
• A statement whereby you request the permanent removal of your name from the prescreen lists that Upstream provides to lenders that send firm offers of credit
• Your signature

Please note the following:

Removing your name from prescreen lists does not affect your ability to apply for or obtain credit.

While your name will be permanently removed from the prescreen lists that Upstream Logix provides to lenders for the purpose of making a firm offer of credit, you may continue to receive offers from sources that do not use Upstream Logix to build their prescreen lists.
